THE POST HOLDER WILL:-
* Process referrals, admissions and discharges to both residential
rehabilitation sites as per referral and assessment procedure
* Work as part of a team delivering an addiction treatment model
through CBT and CRA, MI, SMART Recovery, NDRIC protocols, the national
drugs strategy and other tools; all of which are underpinned by trauma
informed practice
* Engage with and build a professional trusting relationship with
service users; treating them with dignity and respect in order to work
within the ethos and values of Sankalpa
* Induct thoroughly and work cohesively where applicable with Full
and Part time Volunteers / student placements and CE [placements for
the delivery of this service and to be a key participant in the
induction and ongoing training of new staff, graduates, student
placements and community employment participants.
* Participate in a Care and case management model within Sankalpa
e.g. assessment, key working, care planning, transfer of files and
sharing of information internally and agreeing handovers to assist
service users to progress towards their identifies treatment pathway ,
implementing , reviewing the support plan and updating on eCASS and
populating of Outcomes star.
* Will prepare reports, statistics and other information as required
by Sankalpa, to adhere to good record keeping guidelines and usage of
the eCASS, internal case management logs, and Internal IT Systems and
carry out administrative and financial procedures in accordance with
organisation policies.
* Ensure compliance with organisation policies to ensure consistent
delivery of service and maintenance of quality standards.
* Work closely with our external partnerships such as FAST, the
LOFT, the LDTF , probation , HSE and prison services etc to support
service users to achieve the goals set down in their support plan.
PERSON SPECIFICATION:
ESSENTIAL PROFESSIONAL QUALIFICATIONS / EXPERIENCE
QUALIFICATION
_Essential _
* Diploma level in one of the following disciplines: social work,
social care, psychology, addiction studies, counselling, CRA, CBT
addiction studies
_Desirable_
* Degree in addiction studies
KNOWLEDGE
_Essential _
* Understanding of why people become homeless and the needs they
have.
* An in-depth Knowledge of substance, behavioural and non- substance
Addictions
* Understanding of mental health and domestic violence issues.
* Knowledge of the Dublin addiction sector
* Understanding of the needs of groups in the community related to
alcohol or other addictions.
* Understanding of the key working and care planning process.
* Understanding of treatment approaches poly substance addiction
_Desirable_
* Understanding of the issues around alcohol misuse, related health
issues and relapse prevention.
* Knowledge of rights and entitlements of homeless persons.
* Knowledge of complex Trauma
EXPERIENCE
_Essential _
* 2 years paid employment in a relevant care setting with at least
one year experience in direct homelessness and addiction service
provision.
* Experience of facilitating groups and/or workshops.
* Record keeping, report writing,
* Ability to assist with assessment, planned support, key working
and advocacy with vulnerable people.
_Desirable_
* Training Qualification
